Reablement Service Transforms Life After Stroke

Dean, a resident from Hyde, has experienced a profound transformation in his life following a life-altering stroke, thanks to the dedicated support of Tameside Council's Reablement Service. This short-term, free service provided at home has not only changed Dean's circumstances but has also positively impacted his entire family, offering a beacon of hope during a challenging period.

Overcoming Isolation and Uncertainty

Upon returning home from hospital, Dean felt deeply isolated and uncertain about his future, unsure of where to begin his recovery journey. He expressed, "We've never experienced a situation like this, so after I came home, we had no idea where to start or what support we could get." The Reablement Service stepped in to fill this gap, providing essential short-term assistance that has enabled Dean to regain his independence without the need for ongoing care.

Personalised Support and Equipment

Dean's recovery was bolstered by the patient and tailored approach of Reablement support workers, including Kathy, who introduced him to personalised methods and equipment he previously didn't know existed. "I'm so grateful that Reablement's support workers came in. They were patient and provided personalised ways and equipment to help me, most of it I never even knew existed," Dean remarked. His primary goal was to walk again and climb stairs independently, a challenge he tackled with determination.

Through collaborative work with Kathy, other support workers, and physiotherapy, Dean gained the confidence to perform daily tasks such as washing, dressing, and navigating stairs on his own. "Working with Kath and other support workers and Physiotherapy gave me the confidence to start trying to wash, dress and even get up the stairs on my own," he shared. Less than a year after his stroke, Dean proudly reports achieving these milestones, stating, "I'm proud to say less than a year after my stroke, I'm able to do the things that matter to me."

Enhanced Independence and Family Support

To further support his autonomy, Dean utilised a medication carousel, ensuring he could manage his medication independently. Additionally, Dean and his family were connected to other valuable resources, including the Tameside Carers Centre, which offers support for his partner, and Tameside Council's Community Response Service telecare support, providing extra peace of mind for the entire family.

Inspiring Determination and Collaborative Efforts

Tameside Council Executive Member for Adult Social Care, Councillor John Taylor, highlighted the inspiring nature of Dean's journey, stating, "I've had the opportunity to meet Dean and hear his story first-hand. His determination is truly inspiring, and he clearly demonstrates the life-changing and positive impact that Reablement can have." He praised the collaborative efforts, noting, "By working together, it's encouraging to see how hopeful he is for the future. Thank you to the team for working so closely with Dean and his family, and for listening to what is important to them all during such a difficult time."

The Reablement Service is recognised as one of the initial steps of support when individuals engage with adult social care's Early Support and Advice Hub (ESAH). By fostering collaboration, the service aims to help people maintain independent and safe living in their own homes, emphasising a person-centred approach to care.
