New Support Hub at Broomfield Hospital Aims to Enhance Patient Recovery and Reduce Hospital Stays

A new support hub has been established at Broomfield Hospital in Chelmsford, Essex, providing a centralized location for patients, visitors, and hospital staff to access essential community services. This initiative is designed to prevent the need for hospital treatment by linking individuals with local health and wellbeing support as quickly as possible, particularly during discharge from hospital.

Preventing Readmissions and Promoting Home Recovery

The Hub, located in the main atrium of the hospital, offers private conversations with community support services based on-site. Its primary goals include reducing the length of hospital stays and minimizing the likelihood of patients being readmitted by directing them to appropriate help tailored to their personal circumstances. Since its opening in February, the service has already facilitated 40 referrals, demonstrating early success in connecting people with necessary resources.

Julie Smith, managing director at Broomfield Hospital, emphasized the importance of the Support Hub, stating, "This addition empowers people to self-manage their health and wellbeing. Factors such as housing, employment, and lifestyle can significantly impact health, so early intervention with community support may prevent serious illness and urgent care needs." She highlighted that this will benefit the thousands of individuals who visit the hospital weekly.

Comprehensive Services Available

The Hub hosts a range of organizations offering diverse support, including:

  • Citizens Advice Bureau: Provides advice on legal issues, debt, and finance.
  • Essex Wellbeing Service: Assists with smoking cessation, weight management, loneliness, and addictions.
  • Employment Support: Through Essex County Council’s Connect to Work programme and the Mid and South Essex Anchor Programme, help is available for those needing to stay in or return to work due to illness or learning disabilities.

Starting in April, a self-service health check machine will also be available for patient use, further enhancing the hub's resources. No appointments are necessary, as the Hub is staffed by representatives from the various community organizations, making it accessible at any time during hospital stays or upon discharge.

Collaborative Effort for Healthier Communities

This joint project is funded by NHS Mid and South Essex and hosted by Mid and South Essex NHS Foundation Trust. It aligns with local and national objectives to help people maintain better health longer and receive care closer to home. Dr. Matthew Sweeting, executive medical director at NHS Mid and South Essex, commented, "We're pleased to partner with the Trust to bring much-needed community support into Broomfield Hospital. Early access to the right support can reduce avoidable hospital readmissions and emergency care costs, moving us closer to equal health opportunities for all."

The Support Hub not only aids patients and visitors but also serves hospital staff in supporting their patients and their own wellbeing, making it a versatile resource for the entire hospital community.
